I start out with discussing the area of weakness with the student and then try to investigate why this might be the area of weakness. Once this is discovered, I teach the student (through examples) until they understand the topic with which they need help. Then, I observe how they perform without my guidance and see if they have learned to complete problems solo.
Most of my experience is informal with tutoring family members/church members/friends in whatever areas they were struggling.
