In most cases, the student just needs a clearer explanation than his/her book or instructor is providing. Also, I like to create visual examples that make an abstract concept into something more practical. A thorough explanation coupled with examples and practice, practice, practice usually prove to help tremendously. It's also important to listen to your student and to constantly test his/her knowledge of concepts as we progress. My teaching style is helping the student arrive at the correct solution. In problem based courses this is usually done through asking the student what they think the next step should be and guiding them through the process of problem solving.
