I find out what exactly the student is lacking in the subject by asking a few basic questions on the topic and make a judgement of the student's understanding on the topic and then proceed forward. I normally explain the concepts in diagrams so that it makes it easier for the student to visualize the problem and give the student plenty of practice problems for him/her to work on to make their understanding better I have taught over 20 students during my undergraduate and graduate study. My favorite part about teaching is the moment of sudden understanding that you can see in a student's eyes when they've got it. The challenge, of course, is getting to that moment, especially in a subject like math or science. While certain subjects may come easier to some people than to others, I do not believe that anyone is "incapable" of understanding and excelling in any field of study.
