If a student is having a problem, I try to see where there might be a gap in the backround knowledge, then fill it in, or if there is a problem in their processing of the information, I try to correct it. I do this fairly quickly. I'm typically able to tell just from a student's eyes as they look at a problem or how quickly they approach it, if they're having a problem.
