Essentially I find what they know and where do they need help. From there I like to find out something the student has an interest in; thus allowing me to relate subjects to something they find interesting. Students learn better when they see the relevance of the material they are studying to their own lives. In many subject areas this relevance is not obvious, and students may tune out as a result. I always use concrete examples that students can relate to when explaining a concept. For example, when discussing paternalism with college students I will often cite government regulation of tobacco and alcohol use to illustrate how the concept translates into laws that directly affect them.
